    Dear Johnie,

    I hope all is well with you. This is a follow up email regarding the Carers Assessment/Support Plan we completed in Jan 2026, to check that you have been able to use the money for its intended purpose. It would be greatly appreciated if you could please reply to my email to let me know as I need to record this in our records.

    I would also like to remind you that you will need to complete the full application form again after 12 months has passed if you would like to complete another Carers Assessment. You can find the online form on our website www.carersoxfordshire.org.uk, with the link to the Carers Assessment. If you prefer to do the assessment with assistance from one of our staff members, please do not hesitate to contact us on Carers Oxfordshire Carersline on 01235 424 715. Open Monday – Friday, 9.00am – 5.00pm. Alternatively you can send us an email with the Referral form. One of our team will then ring you to arrange a time to do the carers assessment with you over the phone.
